Rydym wedi gwneud rhestr wirio Gwiriad Iechyd Blynyddol, y gallwch ei gweld trwy glicio ar y ddolen isod. Gallwch hefyd ddod o hyd i ddolenni i ddogfennau a fideos defnyddiol eraill os ydych yn darparu gofal iechyd i oedolion ag anableddau dysgu.

Gwiriadau Iechyd Blynyddol

Oes gennych chi anabledd dysgu neu’n cefnogi rhywun sydd ag anabledd dysgu? Trwy ychwanegu eich enw at y gofrestr anabledd dysgu gallwch gael gwell gofal iechyd – a gallech gael Gwiriad Iechyd Blynyddol am ddim.

Beth yw Gwiriad Iechyd Blynyddol?

Cynhelir Gwiriad Iechyd Blynyddol gan eich meddyg unwaith y flwyddyn. Bydd eich meddyg yn edrych yn agosach ar eich sefyllfa – gan wneud yn siŵr eich bod yn iach.

Mae ar gael i’r rhan fwyaf o bobl 14 oed a hŷn ag anabledd dysgu mewn meddygfeydd sy’n cynnig y gwasanaeth.

Nid oes angen i chi fod yn sâl i gael archwiliad iechyd, ond gallwch ofyn i’ch meddyg am unrhyw beth sy’n eich brifo neu’n eich poeni.

Mae’n ffordd dda o ddod i adnabod eich meddyg yn well, ac iddyn nhw ddarganfod mwy amdanoch chi.

Gallwch ddarganfod a allwch chi gael archwiliad iechyd trwy ofyn i’ch meddyg.

Sgroliwch i lawr y dudalen i ddarganfod sut y gallwch chi sicrhau bod eich gwasanaethau iechyd a gofal cymdeithasol yn darparu gwybodaeth y gallwch chi ei deall, gyda’n canllaw hawdd ei ddeall Safonol Gwybodaeth Hygyrch.

Cael gwell gofal iechyd

Nid yw mynd at y meddyg bob amser yn hawdd os oes gennych anabledd dysgu.

Trwy ymuno â’r gofrestr anabledd dysgu gallwch gael:

  • cymorth ychwanegol pan fyddwch yn ymweld â’ch meddyg – gan gynnwys gwneud apwyntiad
  • cymorth i ddeall eich meddyg a’ch gwybodaeth
  • mwy o amser i siarad am sut rydych chi’n teimlo.

Efallai y byddwch hefyd yn gallu cael Gwiriad Iechyd Blynyddol.

The learning disability register

What is the learning disability register?

The learning disability register is a list of people who have learning disabilities. Doctors use it to make sure that people with learning disabilities get the right support in the right ways.

People with learning disabilities don’t always get the right help with their physical and mental health.

Being on the learning disability register tells healthcare staff that they need to adapt their support and care to you and your situation.

If you’re over 14 and on the learning disability register you also get a free annual health check to make sure that you are fit and healthy, and that no problems or illnesses have been missed. This is also a good chance to get to know your doctor better.

What will happen once I’m on the learning disability register?

  • You will get better, clearer support with your health
  • You will be eligible for your priority Covid-19 vaccination
  • You will get extra support when you visit your GP, and for making appointments
  • Staff at your GP surgery will make sure that the information you’re given is clear and easy to understand
  • If you’re 14 or over (in Wales), you’ll receive an annual health check.

How do I get on the learning disability register?

Speak to your GP practice to check if you are on the register. The register is for anyone with a learning disability, including mild learning disabilities. Children with learning disabilities can also join the register.

If you aren’t on your GP’s learning disability register, staff at the GP practice can add your name for you. You will need to make an appointment to tell your doctor that you have a learning disability.
